Terms & Conditions | CME credit | Companion CD-ROM | Contact Info Terms & Conditions Once purchased, a personal account will be activated. This account will be terminated either at the completion of the course (all cases completed at least once) OR at the end of a 60-day period, whichever comes first. This 60-day period starts from the time the login name and password are issued and received. In order to earn CME credit for the course, the user must visit every page of every case. Each course will present a pre-test and post-test to measure effectiveness of the learning tool. After successful completion of the course, the user is also encouraged to complete an evaluation form. Submission of the post-test and evaluation form will generate a request for a CME certificate for the CME activity. A hardcopy of the certificate will be mailed to the user. Also at the conclusion of the course, the user will have an opportunity to purchase the companion CD-ROM for that course. Accreditation This CME activity was planned in accordance with ACCME Essentials and Standards for Enduring Materials. The UCSF Office of CME designates each activity for the maximum number of category 1 credits toward the AMA Physician's Recognition Award, as indicated at the beginning of each module. Receiving CME Credit Note: Each page of a course must be visited to qualify for credit. Companion CD-ROM Note: The companion CD-ROM can only be purchased at the time of completing the eCME course. They are not currently available individually. Terms & Conditions | Contact Info Terms & Conditions After the expiration of the 60-day period, the user will receive an email with a link to an online course evaluation form. Completion of the evaluation form is encouraged to help us better serve you in future courses. Note: Non-CME courses do not award credits.
